Self-Reported Form

For admissions decision purposes, we accept your self-reported GPA (excluding test-optional students) and self-reported test scores.  Therefore, your official high school transcript and official standardized test scores are only required after you have paid your enrollment deposit at Millsaps College.

• If you either did not provide your self-reported information on your application or you have new test scores to report, please be prepared to complete this form and have a copy of your high school transcript and test scores before you begin.

All self-reported information will be verified before enrollment and any admission decision is conditional until official validation of GPA and test scores.  

Please allow 10 business days for processing before you submit any new self-reported data.
